Employment Mentoring • Mentors focus on developing a career plan and employment portfolio, conducting a job search, Networking and mock interviewing, volunteer opportunities in the community, college visits, and job coaching once employment is obtained. • Evaluation – Outcome measures utilize a pre and post assessment of job seeking skills with results showing an 87. 5% increase of skills. Lorain County • Lorain County has had a PATH team since 2006. • The Lorain County Board of Mental Health matches federal funds 1 -3 – For every $3. 00 of federal funds the board matches by $1. 00 – The Lorain County Board of Mental Health contracted with Catholic Charities in July 2014 to run the PATH program Providing Help. Who qualifies? • PATH provides services to people with serious and persistent mental illness, including those with: – co-occurring substance use disorders – experiencing homelessness or at imminent risk of becoming homeless – who are not involved with a mental health agency Providing Help. What is covered • PATH funds can be utilized for : – Outreach services – Screening – Habilitation and rehabilitation services – Referrals for primary health services, job training, educational services, and relevant housing services – Assistance with identifying and securing appropriate housing. Outreach • The biggest focus of the PATH program is outreach. – Outreach is meant to help meet basic needs and build trust and rapport with individuals who do not access traditional services, and connect people with housing, services, and supports. • The PATH team has the ability to reach out to clients wherever they are living. • Traditional case managers are not able to do this. Because: – The person is not a client at the agency – This is not a billable service to Medicaid/private insurance Providing Help. Creating Hope.
Outreach defined • Outreach is face-to-face interaction with people experiencing homelessness. • Outreach takes place on the streets, in camps, under bridges, in temporary motels, shelters, meal sites, and wherever else people might be located. • In active outreach, workers seek out and connect with individuals and families who are homeless. Community Impact of Outreach • Effective outreach lays the groundwork for two important events that impact the community’s infrastructure and resources, including: – The annual Point in Time Count of people experiencing homelessness – The U. S. Census, which occurs every 10 years. • These events require effort and organization, as well as effective outreach. • Lorain County Homeless Taskforce organizes the point in time count. Impact of Outreach for Consumers/Clients • Research over the past two decades has documented numerous positive outcomes associated with outreach, including: – – – • Decreased number of days homeless Improved housing status Decreased psychiatric hospitalization Reduced drug use Improved health and mental health (Olivet et al. , 2010; Buhrich & Teesson, 1996; Bybee et al. , 1994; Fisk, 2006; Goering et al. , 1997; Rosenblum et al. , 2002; Tommasello et al. , 1999) Providing Help. Process of Outreach in SAMHSA Programs • Locate those who would otherwise not being served. • Engage into a trusting relationship. • Screen for housing and other service needs. • Assess health and behavioral health needs. • Provide direct care and services as able. • Refer to housing, health, and behavioral health treatment resources. Housing • Providing immediate housing may not always be the best option. – People have many barriers to maintaining housing. – Barriers need to be addressed prior to placing a person to help the person be successful. › Is someone safe to be in independent housing? › Does the person have the skills necessary to live independently? Housing • There is limited housing in the county. – The Lorain County Board of Mental Health continuously evaluates housing options in the county and seeks funding for housing developments. • The Nord Center manages all housing for mentally ill clients. • The person does not have to be a Nord client, but has to be enrolled in a mental health agency to access the housing. Housing • There is strict criteria to access mental health housing. • A level of need is done to determine what supports are needed for housing. – Group home – Supported housing – Independent living Providing Help. Creating Hope.
