New Partners l NSLHD - Mental health, Oral health l FACS l Legal Aid Steering committee meetings held 6 weekly to oversight implementation and plan services
Mental Health Partnerships l Establish pathway in 2015 l Mental health is conveniently co-located with GP clinic l Referral is not post code specific l Clear diversion and referral pathway to MH from GP l 3 Community workers trained in Aboriginal health assessment and suicide to support this holistic referral.

l Over 70 patients have attended since opening in October 2014 to December 2015 l 76% of patients have received individualised health assessments l 92% have visited the GP
HOLISTIC SOCIAL DETERMINANTS OF HEALTHCARE Bungee Bidgel has addressed many complex and chronic areas of health needs. Over 15% of our 70 patients have received an ATAPS referral An effective model of referral , care and intervention for patients requiring oral health, mental health, housing and legal aid has been provided for over 50% of patients Over 110 referrals to other specialists
