Mississippi State Plan Food Insecurity Physical Activity Social Connectedness Convene food insecurity council or task force to improve food security with an equitable approach during COVID-19 Convene a meeting of existing and new PA partners to educate on physical activity access, healthy equity and COVID-19. Develop partnership with AAA, PDDs addressing social isolation among older adults living in poverty. Conduct needs assessment with food banks across the State to identify gaps and general access issues Draft enhancement to MSDH state improvement plan that support communities on equitable access for physical activity in collaboration with a diverse group of stakeholders Collaborate with the State Unit on Aging to draft a plan to address social isolation among older adults living in poverty 2/22/2021 3
