Tips to Launch S Request Member Registration Forms, ID Cards, and Recruitment Flyers from MCHI at least 2 weeks before launch S Every congregation is different. Some congregations have: S At a service: S Invited MCHI staff to describe the MFHN S Faith leader described why the MFHN is important S Faith leader incorporated a theme of health into the sermon S Made the Recruitment Flyers and Member Registration Forms easily available S Sent mailings S Made presentations at different congregational groups S Brought forms to the sick and shut-in Watch a launch at www. youtube. com/ watch? v=6 hi. Enroll Members S Only adults 18 and older may enroll S The Member Registration Form must be filled in completely S When you receive a Registration Form from a member S Write their Member ID (listed on their ID card) on their Registration Form S Make sure the member’s name is written on the ID card S Give the member their ID card
Provide Care S Every congregation is different S Faith Liaisons may either provide all of the care for the congregant in need or connect them to the appropriate Ministry Leaders S 3 Key Supports S Help connect with Primary Care Provider S Help keep a follow-up medical appointment S Help with prescriptions
