Case Management One Phone Number Program Executive Summary 480 -386 -0234 In an effort to address the communication gap between Bridgeway Health Solutions Case Managers and IPC Post. Acute Providers, IPC Phoenix has developed the Case Management One Phone Number Program for Bridgeway Health Solutions. The One Phone Number Program was designed for IPC Phoenix Providers and the Transitional Care Management Team to work closely with Bridgeway Case Managers to improve care coordination and outcomes by addressing the HEDIS (Healthcare Effectiveness Data and Information Set) performance measures. The One Number Program is the single point of contact for Bridgeway Case Managers to contact IPC Phoenix’s Transitional Care Management Team (TCMT) to address any questions regarding testing, lab results, transferring patients or specific questions for IPC Phoenix Providers. The Bridgeway Case Managers call the one phone number (480) 386 -0234 and the IPC Phoenix TCMT answers the calls and is able to triage over the phone, coordinate patient transfers, gather patient information, track patients and reach out to IPC Providers if necessary to address the questions or concerns of the Bridgeway Case Managers. IPC Phoenix understands how critical having a single point of contact between Bridgeway Case Managers and IPC Providers is to eliminating the communication gap between health plans and providers. Introducing Bridgeway to the Case Management One Phone Number Program and the IPC TCMT will assist Bridgeway in care coordination, improving HEDIS measures, reducing disappointing patient outcomes and lowering the cost of care. 10
