Brent Community Action on Dementia Our Organisation and Governance are straightforward…. 6 Monthly Meetings with Councillor Krupesh Hirani (Brent Health and Wellbeing Board Chair) and Phil Porter (Brent Council) CAD Brent Steering Group Project A Project B “A Dementia Friendly Borough of Brent” These meetings will ensure CAD Brent is linked into the overall health, care and wellbeing agenda in Brent. The CAD Brent Steering Group meet on the final Friday of the month at Brent Civic Centre. The Group is chaired by Danny Maher (CEO, Ashford Place) with secretariat from Naomi Fine. The Steering Group has also done some longer strategy sessions (see Annex B) Project C etc Projects will update at the Steering Group. The Steering Group can also approve new projects/close down existing projects. 4

Brent Community Action on Dementia You can support in the following ways…. 1. Promote our work – Display posters or leaflets for CAD Brent and/or our partner organisations. 2. Sponsor a Project or Fundraise – If you can support a project financially (directly or through fundraising) or in kind (e. g. people’s time) we would be delighted to hear from you. 3. Become Dementia Friendly – Ensure your business or organisation is operating in a Dementia friendly way. This could include Dementia Friends Information Sessions, which cover five key messages that we want everyone to know about dementia, and help people to think what they could do to help. 4. Brent Community Action on Dementia Annex A: A Whole Street of Support Project Purpose • Creating an environment in neighbourhoods where people living with dementia feel safe, confident and involved, playing their part. • Use the Prototyping method of service design and test ideas that might work and change the way neighbourhoods operate. • Initial focus on Cricklewood, now working in South Kilburn. South Kilburn Trust community facilities and connection to other existing groups How will we know it has Succeeded? • People living with dementia report feeling more involved in their neighbourhood. • Examples of change to service providers, community groups and individuals who are more dementia friendly as a result. • Learning how we can energise and align community activities to take action to make their community dementia friendly. Connection to MAPRA residents association. Project Interdependencies Key interdependencies with Peer Support Project – developing resources in neighbourhoods BY peer supporters for use by people living with dementia. Connection to PIE project – developing a range of resources which can form part of social prescribing by GPs in Kilburn and beyond. Brent Community Action on Dementia Annex A: Dementia Care Kilburn PIE Project Purpose Our Partnerships in Innovative Education (PIE) project aims to: • Improve the quality of life for people living with dementia, their families and carers by increasing public and professional awareness Project Lead and Team Who is the lead and who are the other key people involved? Renata Chavda – Project Manager, Dr Justin Sacks – GP Lead and understanding of dementia. • Promote an integrated approach to education and training, spanning primary and secondary care, Community Action Dementia Bren and involving patients and local communities. • Support the development of Kilburn as a ‘Dementia Friendly’ ward. How will we know it has Succeeded? Project Resources Project is funded by Health Education England across North West London (HEE NWL) and is a collaboration with Kilburn Primary Care Co-op Limited, CAD, Brent Dementia Peer Support Project, CNWL, Brent Council, Age UK Brent and Ashford place. Will be evaluated by Imperial College London in September. Grand finale event at Queens Park Fayre. Outputs will include: • Raising awareness and improving knowledge of dementia locally via a series of events within communities especially amongst BAME communities, faith groups and healthcare staff. • A leaflet and poster produced to be displayed at GP practices and public venues. • Workshops delivered to GPs within Kilburn to further improve confidence in recognising and managing dementia. Project Interdependencies - A Whole Street of Support - Peer Support project If evaluation psoitive, will look to roll out to other main GP federations in Brent. • Face-to-face educational sessions with GPs in Kilburn. “A Dementia Friendly Borough of Brent” 10

Project Resources • Dawn Butler (MP for Brent Central) has agreed to be patron of the project – although now subject to re-election. • Brent Mayor’s Charitable Fund How will we know it has Succeeded? • 10 new DCafs in faith / BME communities in Brent. • DCafs fit needs of local population.
