Breastfeeding Surveillance *Completed each time FB’s are issued for infants who are receiving breastmilk at least once a day or more. *Everything except, vitamins, minerals, or medications need to be documented as “something other than breast milk. ”
Surveillance Data • Surveillance data is used to estimate breastfeeding rates and differentiate between “exclusively nursing” and “breastfeeding”. • This data is used to track the effectiveness of our services at reaching Healthy People 2020 goals.

Breastfeeding Surveillance If baby has ONLY received breastmilk, mark “not applicable, nothing given” If baby has been given ANY supplementation other than breast milk (formula, infant foods, sugar water, tube feeding or IV nutrition) identify what day/week/month of life it was first offered.
“Something other than breastmilk” includes: § § § Formula Sugar water Cereal Puree or Solid Foods Cow or Milk Alternative Tube feeding or IV nutrition “Something other than breastmilk” does not include: • Vitamins • Minerals • Medications
Special Scenarios • If data was not correctly gathered during a previous appointment the surveillance date can be changed to the correct data during follow-up appointments. • This screen will capture only the most current ‘Save’. If staff/client are unsure when the first date an infant was fed something other than breastmilk, staff should STOP and ‘Reset’ the screen. Confirm the previous data with Authorized Rep and then proceed with completing the screen appropriately and ‘Save’. •
