BITE-SIZED GUIDES: Refer and monitor an Intervention What can I expect from…? Referral form What is the referral form? Probation practitioners fill in the referral form with detailed information about the service user, their risks, criminogenic needs, additional requirements, and details of the intervention, such as; the number of allocated enforceable days, expected end date, complexity of needs, and desired outcomes. The referral is meant to capture up-to-date information at the time of writing, and is automatically updated when new details are recorded on probation systems. How is it created? After identifying the appropriate commissioned rehabilitative services (CRS), the probation practitioner completes the referral form. They import data from probation legacy systems and review the details. The form includes the most up-to-date information about the service user: ● their personal details such as; ○ given names ○ date of birth ○ contact details ○ risk of harm information with additional comments ○ criminogenic needs with additional comments ○ complexity of service user’s needs (low, medium, or high) ○ additional requirements, such as information about service user’s caring responsibilities, or diversity considerations © Crown Copyright 2020 - HMPPS

● intervention details, such as its expected end date, and the number of allocated enforceable days ● desired outcomes which the service user is meant to achieve with the support given through the intervention service ● probation contact details, including secondary contact details.
