Health Insurance Terminology Allowable Amount Coinsurance Copay • The amount you pay to share the cost of covered services after your deductible is paid. • For example: Tyler’s plan pays 80% of an in-network claim, and you pay 20%. • The fee you pay to share your medical costs, while your insurance company pays the rest. • For example: Tyler’s PPO 750 medical plan has a $25 in-network copay. Deductible Network Premium • The amount you must pay each year to cover eligible medical expenses before your insurance plan starts paying. • For example: Tyler’s PPO 750 plan includes a $750 in-network deductible per individual. • The doctors, hospitals, and other health care providers that insurance companies contract with for services at discounted rates. • For example: Tyler’s dental provider includes their contracted dentists in the Delta Premier/DPPO network. • The amount you pay each pay period in exchange for insurance coverage. • For example: Tyler’s biweekly base premium for employee-only dental coverage is $5. 08. • The amount considered by the insurance company to be a reasonable charge for medical services and supplies based on rates in your area. © Tyler Technologies 2022
Who’s Eligible for Benefits? Eligibility Enrollment Process • All regular, full-time team members who work at least 30 hours per week are eligible to participate in the following benefits: • Health insurance: Includes medical, dental, and vision • Medical concierge • Global surgery • Flexible spending accounts • Basic life and supplemental life insurances • Voluntary benefits: Includes critical illness, accident, pet, and legal • Short- and long-term disability insurance • Employee stock purchase plan* • The benefit plans offered to all team members include: • Employee assistance program • 401(k) with company match • During the annual open enrollment period, eligible team members can enroll/re-enroll in benefits, change benefit elections, add/drop dependents, and waive coverage. • The benefits you elect during open enrollment will be in effect for the following year (Jan. 1 - Dec. 31) — unless you experience a qualified status change. © Tyler Technologies 2022
Who Is a Dependent?
