Select New User Registration from the Portal Login page The link to the Clearinghouse Portal is: https: //apps. ahca. myflorida. com/Single. Sign. On. Portal
Enter all required information as indicated by the red asterisk (*) and select ‘Register’ to continue. Important Notes: • Each user must create an account (this user is typically the CDC+ Representative) • User names/passwords CANNOT be shared • No limit to the number of users per facility • Important! Once your user account is successfully created, select ‘Return to Login’ to access the Clearinghouse results website.
Enter the username and password you created and click Log In
• Select Department of Children and Families/Agency for Persons with Disabilities from the drop down • Click Request Program Access
The name will appear in a drop down selection
The Consumer name should be as registered with APD. Once verified, click Submit Request DO NOT proceed with your user agreement if there is a discrepancy, this may delay your approval.
Important: • The Representative signs both lines of agreement (if the Rep is the User) • Include your driver license • Fastest approval is to scan and email • It could take up to 5 -10 business days for approval
You will receive an email once your agreement has been approved or denied
Review Registration Process • • The CDC+ Consumer is the “Provider” Username and password Do not share your username and password Multiple Program/Provider Access Registration Agreement must be complete Keep your account up to date 5 -10 business days for approval of Registration Agreement
