Introduction The Assessment Alignment Committee develops guidelines for a collaborative approach in the community health assessment (CHNA/CHA) process that leverages the strengths of CCH organizations, allows for aggregate analysis, provides flexibility to address specific organization or sector needs, and fosters collective ability of CCH members to improve community health Meets monthly – 2 nd Thursday at Ramsey County Public Health (St. Paul) Co-chairs: Melanie Countryman (Dakota County Public Health) and Jenny Morman (Fairview Health Services)

2020 Plans Share priority crosswalk and links to CHAs/CHNAs with the Forces of Change participants Meet with staff from the West St Paul YMCA to learn about their Community Healthy Living Index (CHLI) Create a calendar of qualitative data projects that can be shared across CCH membership Make recommendations for conducting qualitative data projects to reduce duplication Work with the Communications Committee on a tool/method to share qualitative data collection results Add new core indicators based on member feedback – infectious disease, mental health and substance use, social connectedness and social isolation Explore capacity and resources within CCH to prioritize 1 -3 key indicators for ongoing tracking and/or deeper analysis Explore data-sharing pilot for non-PPHIC members Share tools to track program outcomes
