ASSESSMENT SKULL AND FACE INCLUDES a. Assessing the eyes and vision. b. Assessing the ears and hearing. c. Assessing the nose and sinuses. d. Assessing the mouth and oropharynx. e. Assessing the neck.
EQUIPMENT q Gloves. q Chart for recording the result of examination. q Well- light warm private room or space.
PREPARATION Inquire if the patient has any history of the following [past history of loss of consciousness, dizziness, seizures, headache, facial pain or injury or has past problem as itching or dandruff].
PROCEDURE Step 1: Introduce self and verify patient's identity. Step 2: Explain procedure to patient. Step 3: Gather appropriate equipment. Step 4: Perform hand hygiene.
PROCEDURE Step 6: Inspect the skull for size, shape and symmetry. Rationale: Normally should be rounded (with frontal, parietal and occipital prominences) smooth skull contour. Step 7: Inspect the facial features (e. g symmetry of structures and distribution of hair). Step 8: Inspect the eye for edema and hollowness.
PROCEDURE Step 9: Note symmetry of facial movement. Ask the patient to elevate the eye brows or lower eyebrows, close the eye tightly, puff the cheeks and smile and show the teeth. Step 10: Rearrange the equipment if used. Step 11: Place the patient in comfortable position. Step 12: Documentation
