Primary Care Behavioral Health • CTC utilizes a PCBH model • The Behavioral Health Consultant (BHC) is available to provide an assessment and interventions at the time of the medical visit. • The BHC is a LCSW or Licensed Psychologist • The right attitude-Non Judgmental, compassionate, begin where the patient is. • Differs from traditional behavioral health.
Difference between PCBH and Psychotherapy Primary Care Behavioral Health • • • Same Day Interventions 15, 20, 30 minutes 30 to 40 consults a week Open Access Broad focus-multiple psychosocial factors Psychotherapy • Referral wait times- days to months • 40, 60, 90 Minutes • 25 to 30 sessions a week • Closed Case Load • Mental health focused
Psychosocial factors that negatively impact health and well being • • • Coping strategies and patient strengths Mental Health and Substance Use Income and Housing Social Support, Family and other Relationships Self-esteem, Confidence and Motivation
BHC Interventions • Motivational Interviewing- Exploring internal reasons for change • Problem solving and Coping skills • Relaxation and mindfulness • Cognitive restructuring • Emotional Awareness and management • Identify the next step in their journey
Care Coordination • Identifying the resources necessary to support patient in their wellness plan • Connecting patient with necessary supports • More then simply giving patient a pamphlet or a phone number • It is about connection and communication.
