Learning Objectives For Community Partners/non profits: • To understand the benefits to having funders at the table early-on in a project planning process. • To learn the processes involved in starting and maintaining collaborations with funders. • To understand how grantmakers can actively take part in local public health community efforts.

Learning Objectives For Funder/Donors: • To understand the benefits of involvement in local public health planning endeavors. • To collectively explore ways to partner with public health-focused nonprofits, share work with other foundations, and communicate the need for collaboration to our boards. • To identify methods by which grantmakers can share ideas and best practices that will increase our creativity, leverage our dollars and strengthen the practice of public health.

Roundtable 1 Starting Strong in Illinois Collaborative Program Goals : • To build and sustain a multi-sector coalition, including public health advocates and providers and the region’s health foundations, to address the ACA in Illinois. • To create a statewide roadmap that coordinates policy, advocacy, education and outreach for health care reform implementation in Illinois, with a focus on the most vulnerable populations. • To facilitate the exchange of useful and strategic information about the ACA between public, private and community-based agencies.

Roundtable 3 Diabetes Healthy Outcomes Program: Healthy Outcomes for Uninsured Diabetic Patients at Hamilton Health Center Program goals: • To establish a strong community partnership/collaboration between the Highmark Foundation and Hamilton Health Center (HHC), a Federally Qualified Community Health Center (FQHC) that has broad impact. • To improve diabetes outcomes among uninsured diabetics by providing access to a seamless continuum of comprehensive, coordinated health care and supportive services at a FQHC. • To leverage the Highmark Foundation’s funding to demonstrate new and sustainable ways to create a successful and replicable model of diabetes care.

Roundtable 4 A National Funder Collaborative for Gun Violence Prevention Program goals: • To educate public health funders about a new donor collaborative dedicated to reducing gun violence. • To share information about current efforts underway focused on public policy approaches to reducing gun violence. • To highlight a successful model for reducing gun violence through research, coalition development and policy advocacy.

Roundtable 5 Bringing Voice to Communities through Convening Program Goal’s: • Identify the role of convening as a way to give voice to issues • Understand broader definitions of community than geographic definitions • Describe how results from community convenings can be positioned to drive policy and program change
