Case Study: There is evidence that hygienic cleaning has a role in the control of hospital-acquired infections. This study aimed to evaluate the potential impact of one additional cleaner by using microbiological standards based on aerobic colony counts and the presence of Staphylococcus Aureus including meticillin-resistant S. Aureus. Method: An additional cleaner was introduced into two matched wards from Monday to Friday, with each ward receiving enhanced cleaning for six months in a cross-over design. Ten hand-touch sites on both wards were screened weekly using standardised methods and patients were monitored for meticillin-resistant S. Aureus infection throughout the year-long study. No changes in antibiotic policy; no additional hand hygiene interventions nor formal hand hygiene audits happened during the period of the study. October 2012 29
Results: Hygiene failure reduction: 50. 3% (aerobic colony counts > 2. 5 cfu/cm 2 ) Number of new MRSA infections: Routine cleaning: Enhanced cleaning: expected actual Difference: October 2012 9 13. 1 4 (375 MRSA patient days) (= (9/327) × 475 MRSA patient days) 9 infections less ! 30
Cost Analysis Cost calculation based on annual numbers Extra cleaner: Consumables: € 15, 400 € 1, 370 Cost of one hospital-acquired surgical site infection caused by MRSA: € 11, 2301 9 infections less per annum/ward: € 101, 070 Annual cost savings/ward: € 100, 000 + 1.
