American Family Financial Association How To Get Your Screening Ø Complete the Scope of Appointment (SOA) This form gives me permission to speak with you regarding the Screening process; To ask you a few questions regarding your family’s health history as well as your own; Allows me to check your Medicare Plan. Ø Connect you to a Clinical Admin The Admin will get the proper documents completed and Connect you to a Clinition for final screening to determine if you will qualify for the Genetic Test. Ø Collect DNA – Cheek Swab If approved, the Clinician will direct me to do a Cheek Swab. I will send your DNA to the lab for testing. Ø Your Doctor will share their findings with you and your PCP. It takes approximately 2 weeks to get the test results.
