ALCOHOL SCREENING PROGRAM • • • • 1. Marine shows up to work under the influence of alcohol: -Two Choices: -Do you want to use BAC as evidence to NJP the Marine -Do you just want to have a “COMPETENCE FOR DUTY EXAMINATION” (NAVMED 6120/1) done on the Marine. 2. You want to use BAC as evidence to NJP the Marine: -Immediately fill out AFMES FORM 1323 (Toxicology Report) and the NAVMED 6120/1 (COMPETENCE FOR DUTY EXAMINATION) -Once filled out, take it to the Commander for Signature. -Ensure the forms have: -Correct UNIT address: -Marines CORRECT SSN: -Incident details (BOTH AFMES FORM 1323 and NAVMED 6120/1. ) -A Permissive Authorization for Search and Seizure (PASS) or Consent form OPNAV 5580/16 must be completed to draw blood/urine. • • •

ALCOHOL SCREENING PROGRAM • • • • -Once both forms have been signed take the Marine to the LAB at Naval Hospital Camp Pendleton. -Ensure the Marine has his ID with him. -Check the Marine into the lab and give the lab technician the forms. -The lab will retain AFMES FORM 1323 for chain of custody and mail results. -Once the labs are complete take the Marine to Medical for a “COMPETENCE FOR DUTY EXAMINATION”. -**Ensure you have this signed prior to going to the lab so that you save time. ** -Fill in lines 1 -12 and check the box for laboratory analysis. -You can have them seen at either Naval Hospital or the Area CAS/BAS for evaluation. The hospital may send you to Area medical if they are too busy with urgent patients. -The Marine will then get seen by a Medical Officer -post the results

You just want a “COMPETENCE FOR DUTY EXAMINATION”. -Fill out NAVMED 6120/1 and have it signed by the Bn Commander. -Fill in lines 1 -12 and DO NOT check the box, (line 7) for laboratory analysis. -Take the Marine to the Area CAS/BAS for evaluation. -The Marine will be seen by the Area CAS/BAS Medical Officer. - Medical will return the original document to you. - Post the results.
