Healthy Activities Eligible For Rewards $10 For Completing a Health Risk Screening. (One per calendar year. ) $10 Per Hb. A 1 c Test for Diabetic Member. ($20 max. ) $50 Annual Comprehensive Diabetes Care. Age 18 -75. Must complete all of the following once in the calendar year. If 2 Hb. A 1 c tests are completed in calendar year, then total $70 max. Hb. A 1 c test Kidney screening Dilated eye exam $10 BONUS for the Following Immunizations. Age 12 -15 months. MMR VZV $10 Per Infant Well Care Visit Up To 15 Months Old. Visits are recommended at 3 -5 days old, before 30 days old and at 2, 4, 6, 9, 12 and 15 months old. ($60 max. ) $10 Annual Well Care Visit with Child’s Primary Care Doctor.
