Project Overview �Adopting Bedside Reporting & Utilization of Standardized Report Forms in SBAR format �Joint Commission Goal �Improving Communication between caregivers at shift change �Improving patient satisfaction and safety
Project Goals: � 1. Transition RNs to perform bedside report during shift change while utilizing handoff shift form by June 30 th. � 2. Improve patient satisfaction scores as evidence by patient perception of adequate communication from nursing staff through informal survey by July 20 th. � 3. Implement the use of a standardized handoff report form following SBAR format by June 30 th.
Project Objectives � 1. Create and develop a handoff report form orthopedic specific by June 10 th. � 2. Educate RNs on importance of effective communication through walking rounds to improve patient satisfaction and patient safety by June 15 th. � 3. All RNs to perform bedside report utilizing standardized report form by June 30 th. � 4. Will perform weekly informal surveys with patients and staff on perception of communication from the patients, and staff satisfaction with transition to bedside report. Informal surveys will begin July 7 th and will continue through July 28 th.

Process, Progress, and Completion of Project � Stage 1: Develop a Standardized form in SBAR format Obtain Binder for Nurses station to hold report forms Organize binder in a chronological order by each room number � Stage 2: Gather Evidence-Based Practice regarding outcomes related to bedside report and standardized report forms Develop a read and sign on benefits of bedside report and the process to perform Send email to all RN staff on the initiation of bedside report with a read-receipt � Stage 3: Ensure all RN staff is aware of the transition to bedside report and utilization of standardized report forms Ensure that all RN staff has read the educational material and all questions are addressed � Stage 4: Designate charge nurse or management to randomly survey patients Record data in data form provided Create data form Compare data at the end of project Inform staff of results Adjust or change as necessary
