Access to ADAPTABLE Web Portal To request a site user account: § Contact DCRI Coordinating Center § Provide NAME, EMAIL ADDRESS, ROLE of site coordinator needing access You will receive an email from Mytrus confirming your user account set-up Go to ADAPTABLE Web Portal to login: Adaptablepatient. com Note: You can update your email and password from the user profile icon (upper right hand corner of screen) at any time after logging in
Click on LOGIN
After Logging In. . List of site participants will be displayed Navigate to the participant you want to update Click on DETAILS to get to the visit pages Enter missing data – Early Visit Only! § Aspirin Question § Visa Gift Card Question Update the patient contact information if needed
After Logging In. . List of site participants with ability to sort alphabetically user profile where you can update your email address/password Search function allows you to search on last name or first name or golden ticket number Clicking on Details takes you to that participant’s visit forms
Navigate back to participant List (can also use back arrow) Assigned Dose and visit schedule Current Level of Participation (e. g. Full Participation, Withdrawn) Visit Ruler STEP 1: Click on due visit with red circle to get to the forms in that visit STEP 2: Click on History Form to update aspirin question STEP 3: Click on $25 VISA Gift Card to update gift card question
Update the two “Early Visit” Questions Non-internet subjects have two remaining questions to complete for their “early visit” (aspirin and gift card) Click on the Early Visit icon located on left hand side of screen and select the History Form to get to the aspirin question § Questions 1 & 2 should have already been completed by the participant at time of randomization § Question 3 is the current aspirin usage question and must be filled out by the site coordinator § Confirm with the participant that he/she has started taking the assigned aspirin dose. Enter the dosing information and the enteric coded question. § If participant has not started taking aspirin and does not plan to start taking aspirin, mark the appropriate reason in the portal Click on $25 Visa Gift Card Form to answer whether the participants wants to receive a $25 gift card Update participant contact information if needed
Early Visit, History Form: Aspirin Question (cont. ) NOTE: The other questions on the History Form should have already been filled out by the participant at time of randomization It is important to follow up post randomization to confirm the participant has purchased the assigned aspirin dose and has started taking aspirin. This is the 3 rd question on the History Form.
Aspirin Question (cont. ) If participant had started taking aspirin but then stopped, select NO and indicate the reason from the drop down options or free text field If participant never started taking aspirin post randomization, select “I never started taking aspirin” option
Aspirin Question (cont. ) Once all the responses are entered, click on SUBMIT. NOTE: Once you SUBMIT, the response cannot be changed
Early Visit, $25 VISA Gift Card Form Ask participant if he/she would like to receive a $25 Visa gift card as a thank you for participating in the study Record the participant’s response in the Visa Gift Card form Note: It takes from 6 -8 weeks to receive the gift card in the mail

EARLY VISIT – Visa Gift Card Form This question cannot be answered without a mailing address. If contact information is blank, the system will prompt you to enter the missing address information before proceeding to the Visa gift card question. Click SUBMIT when done

Next Steps After the site coordinator updates the two missing questions (aspirin and Visa gift card) in the Early Visit, the Early Visit will be marked as “complete”. The DCRI Call Center will follow up with all non-internet subjects when their subsequent visits are due. Site coordinators should not enter data on any of the subsequent visits
