Access for the current semester, you will be required to read and electronically sign any required forms that the Disability Center has pertaining to specific accommodations. 2. You will see a list of the classes you are enrolled in. Click the checkbox next to those classes in which you plan to utilize some or all of your accommodations. Click Step 2 below the yellow box titled ‘Rights and Responsibilities’.

Steps to Generate your Accommodation Notification Letters (cont)… You will be taken back to your Overview page where the ‘Printing Faculty Notification Letter in PDF’ dropdown menu will appear. You will select the class and click on ‘Generate PDF’. A file will download onto your computer and you can then print it and hand deliver to your instructor(s) or attach into an email and send it to them. *Remember, if you email your letters you will still need to find time to discuss your accommodations with your instructors.

Accommodation Plan Modification Request Form If you would like to request additional accommodations other than the ones you have already established with your Access Advisor you will need to complete the ‘Accommodation Plan Modification Request Form’. The request form is located in the “Home” section of your my. Access account. After you’ve submitted the form, please allow 3 business days for your Access Advisor to contact you about your request once they have reviewed it.
