Accident Reporting and Investigation OHR Management OHR Commanders will: • Emphasize Importance • Prompt Reporting and Investigation • Separate OHR and flight violation systems • Forward OHR to higher level when needed • Correct Hazards at the lowest level • Review and Sign Form OHR

Accident Reporting and Investigation OHR ASO Duties • Actively Promote • Receive, analyze and recommend control options (investigate) • Prepare for verbal reports • Ensure forwarding actions: • 20 working days returned back to originator OR • update every 10 days (in writing) OHR

Accident Reporting and Investigation OHR An Effective OHR Program • Emphasize that the OHR and a flight violation report are two separate systems • Target corrections on systems defects rather than simply fixing operating errors • Get command involvement! OHR

Accident Reporting and Investigation OHR Submitting Hazard Reports • Military or civilian may submit • Anonymity is possible • Not required in lieu of accident or quality deficiency report (QDR) • In-Flight; report via radio and/or upon landing • Only report aviation hazards OHR
