Accessing Office 365 Forms Login to Office 365 and access Forms from the Apps area.

Differences between a form and a quiz • You have two options from the forms home page: New Form or New Quiz. • A form can be used to gather information from those filling out the form. • A quiz is used to test knowledge of information and has preset correct answers. • Choose which suits your purpose and click “New”.

Office 365 Forms-Title and Question Types • There are four kinds of question types on a form: • • Choice-multiple choice options Text-Open ended text response Rating-Star or Number ratings Date-choose date from calendar • Add questions to reflect the information you would like to gather on your form. You can allow multiple answers and you can require a question be answered by used the toggle switches at the bottom of each question. • Use the “Theme” button at the top of the screen to choose a color scheme/theme for the form.

Office 365 Forms-Quiz option • To create a quiz, click on “New Quiz”. • There are four kinds of question types on a form: • • Choice-multiple choice options Text-Open ended text response Rating-Star or Number ratings Date-choose date from calendar • Note you will have the ability to choose a correct answer in quiz mode. • You will also have the ability to assign points for grading purposes for each question of the quiz.
