Academic Progress Standards Algorithm: Warning, Probation, Dismissal, & Appeal Process Academic Warning Letter sent to student indicating “Academic Warning” Status. 1 st Quarter Only Cumulative GPA and / or Professional GPA < 2. 0 Students expected to meet with Academic Advisor to develop and maintain an Academic Plan and receive information on academic and other resources. Both Cumulative GPA and Professional GPA of ≥ 2. 0 must be achieved by end of the following quarter or student will be placed on probation. Academic Probation 2 nd & Subsequent Quarters Cumulative GPA and / or Professional GPA < 2. 0 Failure of 2+ core courses Letter sent to student indicating “Academic Probation” Status and conditions of probation. Students expected to meet with Academic Advisor to develop and maintain an Academic Plan and receive information on academic and other resources. Cumulative GPA and Professional GPA ≥ 2. 0 Cumulative GPA or Professional GPA < 2. 0 AND Quarter UW GPA and Quarter Professional GPA > 2. 5 Cumulative GPA or Professional GPA < 2. 0 AND Quarter UW GPA or Quarter Professional GPA < 2. 5 Off Probation Appeal to Dean’s Office for Review of Dismissal Action and Reinstatement Letter sent to student indicating “Off. Academic Probation” status. Student submits letter to Academic and Professional Standards Progress Committee including: Continue Probation Letter sent to student indicating “Continued Probation” status. Appeal Process Academic Dismissal for Low Scholarship Petition Denied Letter sent to student indicating dismissal from the program. School notifies Board(s) of Pharmacy that the student is no longer in the program. Failure of a Core Course a Second Time Student with 2+ failed core courses who fails an additional core course Algorithm based on the UW Student Governance and Policies found at: http: //www. washington. edu/admin/rules/policies/SGP/Schol. Reg. CH 107. html Note: APPEs use the course prefix of PHARMP (Gold Curriculum) or PHARMPR (Purple Curriculum). Although the numbers vary to designate individual practice experiences, APPEs are considered the same course. According to School approved policy, a student who fails the same course twice will be dismissed from the program; therefore, a student who receives a grade of No Credit in a second APPE, regardless of course number designation, will be dismissed from the Pharm. D program. Petition Denied 1. Request for review and reinstatement 2. Statement describing the cause of academic difficulties 3. Academic Plan to improve grades the following quarter Petition Approved Student does not Appeal Student Appeals to Dean Decision of the Dean is final Petition Approved Student Reinstated 1. Student re-enters on academic probation 2. Grades from subsequent courses may not be used to raise Cumulative or Professional GPAs remain the same as when dropped from the Pharm. D program.
