No. Version: 1. 0 Revision: 0. 0 Effective Date: 02. 11. 2015 Description 1 The Requestor needs to make a request through email or service desk. 2 Based on the request, the Service Desk Manager/Lab Engineer/Technical Assistant/Technician to coordinate and decide repair mode. 3 and 4 If it could be solved internally, Service Desk Manager/Lab Engineer/Technical Assistant/Technician to distribute the job to respective person-in-charge. If outsource job recommended, Dean’s approval shall be sought. If the Dean’s disagreed, then the Service Desk Manager/Lab Engineer/Technical Assistant/Technician shall further discuss with the Dean for solution. 5 Manager to check for the availability of budget allocation. 6 If no budget allocation, Manager need to do budget virement or request for extra budget. 7 Manager to proceed with procurement process. 8 Job to be carried out either by the internal or external parties based on recommendation and approval. 9 Both In House and Outsource job completed must be verified by the requestor to ensure that the job is done according to the requirements. The requestor is required to update the status in service desk and verified by the Lab Engineer/Technical Assistant/Technician. Note: In the absence of the requestor, the verification could be done by the administration staff. PAGE 2 of 2 Form/Report
