What is LISS Low Intensity Support Services (LISS) are support services, funding and resources up to $2, 000. LISS is designed to help a family provide for the needs of a child or an adult with developmental disabilities living in the home or to support an adult with developmental disabilities living in the community. 3

How can I be sure that I fall under the eligibility requirements? � "Developmental disability" means a severe chronic disability of an individual that: � (a) Is attributable to a physical or mental impairment, other than the sole diagnosis of mental illness, or to a combination of mental and physical impairments; � (b) Is likely to continue indefinitely; � (c) Is manifested in an individual younger than 22 years old; � (d) Results in an inability to live independently without external support or continuing and regular assistance; and � (e) Reflects the need for a combination and sequence of special, interdisciplinary, or generic care, treatment, or other services that are individually planned and coordinated for the individual. � For a list of other detailed definitions that relate to the LISS Program please visit the following website: http: //www. dsd. state. md. us/comar/getfile. aspx? file=10. 22. 01. htm 4

Who is eligible for LISS To be eligible for LISS assistance the individual must meet all of the following guidelines: �be diagnosed with a developmental disability prior to the age of 22 (Only official and specific diagnosis will be accepted. ) �be a resident of Maryland �not currently receiving any DDA services (Community Pathways, Supported Employment etc. �be on DDA’s waiting list 5

What does LISS cover � Housing Assistance � Eviction Assistance (with a court ordered eviction notice still in affect at the time of processing) � Utility and water disconnect (with a cut off notice still in affect at the time of processing) � Deposits, initial only � Personal Care � Recreational Memberships � *Respite Services � Cannot exceed a total of 45 days � Cannot run more than 30 consecutive days � Cannot be paid to a parent or guardian of LISS applicant. � Specialized Equipment � *Therapies: (not covered by insurance) � Music � Art � Horseback riding � Transportation Costs � Vehicle Modifications 7

What LISS does not cover Here are some things that LISS cannot pay for which include: � Case management � Gift cards � Presents – like you would get for a birthday or during Christmas � Rent � Toys � Tuition � Vacations � Gas for a car � Car tires � Car registration � Car violations like tickets and fines � Items paid for by private insurance or Medical Assistance or other programs � Basic household appliances/furniture. � Services not covered in Fiscal Year 2015 (with an exemption of camp services. ) 8

5 Easy Ways to Submit the Perfect LISS Application 1. Complete the LISS request form �Answer all questions as they pertain to the applicant �Applicants over 18 – must provide a copy of their medical assistance card or proof of application for medical assistance �If you have more than one request list them in order of importance �Sign and date the application �Only CURRENT applications will be accepted on or after January 1, 2015, (faxed applications will not be processed). Keep in mind that the individuals who receive funding will be selected by a lottery system; therefore, there is no rush to get your application in on the first day but it should be received by January 31, 2015 9

The Difference Between Similar Services Respite vs. Daycare Respite Services • Can be provided in the home • Can be provided by a non licensed professional (Grandma, Uncle, Neighbors) • Parent or Legal Guardians may NOT provide respite care services • Supporting documentation needed W-9, Independent contractor, ID, & Social Security Card Day Care/ After School Services • Must be provided at a LICENSED facility. • Must be provided by a LICENSED Daycare Professional. • Facility and provider must be registered and certified under the state of Maryland • Supporting documentation Tax-ID Number & licensing information ! Please Note: once you have entered a specific request for either daycare or respite you will be held within that category for the remainder of the fiscal year. 12

Recreational vs. Therapeutic Recreational Services • Can be provided by any organization in the state of Maryland. • Services must fall within the fiscal year. • Documentation Needed: -Invoice or estimate of coast that is specific to the LISS applicant. -dates of service Therapeutic Services • Must be a LICENSED Therapeutic facility in the state of Maryland. • Must be provided by/Supervised by a LICENSED Therapist. • Must be a specific therapy to assist with disability (OT, Speech, PT. ) • Documentation Needed: -Letter of recommendation from a licensed health care professional (With licensing number on the letter) -Licensing information for therapist providing the service Please Note: once you have entered a specific request for either Recreational or Therapeutic you will be held within that category for the remainder of the fiscal year. If the Vendor can not provide the necessary documentation to be called a “Therapeutic Service” you will be asked to choose another licensed therapeutic provider. 13

Proof of Developmental Disability Those who have a severe, chronic disability which is caused by a physical or mental condition, diagnosed prior to the age of 22, other than a sole diagnosis of mental illness verified by: � � Individualized Education Program (IEP) full “APPROVED” report or Individual Plan (IP) Medical evaluation with the Specific developmental disability diagnosis (documentation that states multiple disabilities, other health impairments, specific learning disability will not be accepted without further documentation. ) Psychological report Department of Heath & Mental Hygiene letter for eligibility 14

Please be sure to check the DDA website as often as possible for changes an updates. � As it stands today when applying for summer camp you can be reimbursed as long as � Services fall within the fiscal year � The service must have been paid for between the months of April, May, & June, and occur during fiscal year 15 ( July 1, 2014 - June 30, 2015) � The camp provider must be LICENSED through the state of Maryland to provide “CAMP SERVICES” � Department of Health & Mental Hygiene camp certification; � Accreditation by MSDE; � Approval of a state agency; � OR have a professional licensed to provide services (for summer programs such as speech "camp") 15

Invoice for service/support request �Invoice from vendor for services to be delivered between July 1, 2014 and June 30, 2015 �Completed order form �Completed registration form with cost sheet attached �Letter from the vendor indicating service or support being provided, dates of service, and total cost Please Note: All services must be completed by June 30, 2014 18

Independent Contractors �If you choose to use an individual person to provide a service

Independent Contractors �If you choose to use an individual person to provide a service or support (i. e. respite care, day care, etc. ) this individual will be considered an independent contractor. Below is the information that is required from all independent contractors: � Legible copy of identification (Must be age 18 or older); � Legible copy of Social Security card; � Completed W 9 tax form ; � Completed Sole Proprietor’s form; � Invoice with projected dates of service and the daily rate of pay; � If approved, biweekly submission of timesheets is required; � Independent Contractors paid over $600. 00 by December 31 will receive from Maryland Community Connection a 1099 form for tax filing purposes. �Independent Contractors cannot be a parent or guardian. 20

LISS Approved Payment Process � If you are approved for an item that can be purchased online (Ipad, Adaptive Equipment, Adaptive Clothing) the LISS agencies ensure the shipping of all tangible items directly to the applicant or family, and bill the provider or vendor. *Please be sure to include shipping cost & warranty for items your LISS Application. Items will be shipped to the address located on the LISS application. PAYMENT FOR RE-OCCURRING EXPENDITURES � � � � � Services that re-occur will not be paid upfront in one lump sum. You must submit an invoice/timesheet for expenditures that re-occur through out the fiscal year If you submit an invoice/timesheet on the 1 st of the month it will be processed on the 15 th of the month. If you submit an invoice/timesheet any other time throughout the month it will be processed on the 1 st of the month. Processing for invoices/timesheets only occurs on the 1 st and the 15 th of the month. To ensure LISS services are individual, or family directed, LISS pays for ongoing services, such as Respite Medical Therapeutic Supports Daycare 21

All applications MUST BE in the OFFICE by January 31, 2014. Maryland Community Connection will not accept applications that are: � Post marked on the 31 st but delivered at a later date � Dropped outside the front office door � Faxed in �LISS reimburses for approved services already rendered or paid for only if they were received in the 2015 fiscal year.
