Objectives of this lecture: Recognize normal pleural anatomy & physiology in children. Identify different pleural diseases in children Manage pleural diseases in children
Pleural Physiology: Fluid circulation Pleural fluid 0. 3 ml/kg In continuous circulation Negative pressure Normal Pleural Fluid; ph 7. 6, prot <2 gm, few cells, LDH <50%
Function of pleural space The pleural space couples the lung with the chest wall and provides an integrated respiratory system.
Causes of Pleural Effusion Transudate High systemic venour pr; (eg. HF) Low oncotic pr; (hypoprot. ? ? ? ) Exudate Increased capillary permeability; (capillary wall insult) eg; pulm infections, pleural infections, adjecent inflammation, CT diseases, malignancy. Iatrogenic; trauma Chylothorax
What is empyema? = presence of pus in pleural cavity Occurs with severe infections Infection initiates massive inflammotory cellular influx, activation of coagulation cascade, reduction of fibrinolysis & massive fibrin deposition in pleural space. Very thick pleural collection blocks lymphatic pores and prevent pleural drainage.

TTT Cause Intercostal tube application; indicated if – empeyma (of any amount) or moderate to severe pleural effusion (what is the care of the tube? ) Fibrinolysis Video assisted thorascopic Surgery (VATS) Open surgery; decortication operation
Antibiotics All cases should be treated with intravenous antibiotics and must include cover for S pneumoniae. Broader spectrum cover is required for hospital-acquired infections, as well as those secondary to surgery, trauma and aspiration.
Pneumothorax Accumulation of extrapulmonary air within the chest May be primary or secondary May be sponteneous, traumatic, or iatrogenic Onset is usually abrupt and severity of symptoms depends on the extent of collapse lung

What is the meaning of Bronchiecatsis?

Characteristic central bronchiectasis 2/2 ABPA
Note characteristic location in the upper lobes and superior segments of lower lobes
Pathogenesis Always starts with chronic infection of bronchi Cause of this chronic infection is one or more of the following; Resistent Organism infection Stasis of bronchial secretions (physical/functional) Immunocompromisation (host defence defect; congenital; acquired or iatrogenic)

This chronic infection will result in chronic inflammation of the bronchi
Inflammation Characteristics across etiologies: - Persistent - Neutrophil dominant - Pro-inflammatory cytokines (IL 8, IL-1, TNF-a) - Low anti-inflammatory cytokines (IL-10)
Sequence of Persisten Chronic Airway Inflammation Altered Airway Milieu Proteolytic damage -e. g. cleaved receptors -impaired macrophage function Oxidant stress -low antioxidants associated with worse disease -dysregulation of signaling and cellular function
