Connected Care User Experience Increasing Safety…. Improving Care Quality…. Releasing Time to Care… “……… not a known diabetic and feels unwell can potentially be a life threatening scenario on our hands. I accessed Connected Care and saw that she is not diabetic which helped me decide an ambulance was needed” “I have found having access to Connected Care really useful. I use it primarily to check that a woman’s pregnancy is still viable before we contact her to offer an antenatal contact. ” “Medication and history is available, saving time contacting family or calling GP for information“ (Community Health Visitor) (Oo. H GP) “I found that the patient has osteoporosis and was on a drug that is contraindicated in elderly patients and can cause bone fractures especially in patients with osteoporosis, this was vital information that the GP had omitted and could have had a detrimental effect upon the patient if not acted upon swiftly. By activating connected care I had a fuller and safer picture of the patients medical history” (Diabetic Specialist Nurse) “It is not unusual to see patients with Cognitive Impairment who are on their own, they are unable to give an accurate account of their information…I can access Connected Care, see important information such as their problems and medication and safely treat them without further delay” (ED Doctor) “One in particular was being able to check on the welfare of the client who had been on our waiting list for allocation and had we not been able to get in contact ………. was able to check on Connected Care and found out that the client was in hospital and the ward was available which made things very clear when contacting hospital for updates” (Local Authority Duty Coordinator) “Young epilepsy patient came in to A&E on Sunday afternoon, person was drowsy and unable to give drug information or history, because I had access to their Connected Care record I was able to give best care rather than standard emergency care. ” (Consultant Geriatrician) (ED Clinician) “Sometimes we spend the first 24 hours of an admission just finding out what somebody somewhere knew already and the next 48 hours doing investigations that have already been done!” (Consultant Physician) “When a patient moves from another practice it takes some time for their notes to be summarised and sent across to the new practice. Connected Care will allow us access the record straight away and give the appropriate care , it saves us having to call another practice for information to be faxed across. ” (GP) “Other services having access will free up Reception staff (calls to fax information). This is more secure – there is a worry about faxing – as no guarantee that fax has been received by right person” (GP)
