Process: Log into the Ivisions Portal • Benefits Tab • HR Benefits Enrollment Navigate through each screen completing as directed. To accept current elections, choose “Next”. • FSAs require re-enrollment if applicable
Post Enrollment: Turn • in STD Form if applicable Due by 5: 00 pm on 5/30/2019 to Cyndy Nelke, CEE 5 th Floor or Fax # 602 -274 -0484 • Failure to turn in Form will void OE Election Initiate EOI Process • www. standard. com/mhs • Denial by carrier or failure to complete EOI will void OE Election
