What Didn’t Work � Access to Care � $101, 360 grant from Boone County � Assisted citizens in finding a medical home � Learned that unless financial barrier can be addressed, helping with all the other barriers is non effective � Returned majority of grant funds to Boone County for re-allocation

Safe, Healthy, and Affordable Housing: How do we create a community where safe, healthy, and affordable housing is accessible to all? Goal: In coordination with existing systems, ensure all housing is safe, healthy, affordable, code compliant, and energy efficient Strategy: Identify and implement policies that guarantee everyone has access to safe, healthy, and affordable housing
Safe, Healthy, and Affordable Housing: How do we create a community where safe, healthy, and affordable housing is accessible to all? Goal: Develop communitybased strategies that promote personal empowerment to improve access to safe, healthy, and affordable housing Strategy: Develop and promote tools and information to make safe, healthy, and affordable housing choices
Basic Needs: How de we create a community in which everyone’s basic needs are met? Goal: Reduce barriers to meet basic needs Strategies: Increase access to transportation Increase access to healthy food Simplify access to local social services Reduce the stigma of asking for help
Mental Health: How do we create a community in which everyone’s mental health needs are met? Goal: Increase awareness of mental health issues, resources, and services for all Strategies: Provide health professionals, law enforcement, and first responders education to identify signs of mental health and substance use issues and referral resources Develop and implement a media campaign for the adult population to reduce stigma
Medical and Dental: How do we create a community in which everyone can achieve their optimal level of medical and dental health? Goal: Identify and reduce barriers in access to health care Strategies: Increase awareness among Boone County residents, providers, and referral agencies of currently available health resources and how to access them Promote the use of community health workers from underrepresented and underserved communities
Medical and Dental: How do we create a community in which everyone can achieve their optimal level of medical and dental health? Goal: Improve health behaviors and environments Strategy: Implement community -based interventions to promote health and reduce health disparities
