2018 INFECTION PREVENTION ADVOCATE PROJECT Project Title UVMHealth. org/Med. Center Department or Unit Name PROBLEM STATEMENT Type in your problem statement. What specifically was the problem you were trying to address? STRATEGIES LESSONS LEARNED Describe what you did. List the strategies and then describe each one in some detail Type in what you learned. For example, things that worked well, things that did not work well, what you might do differently if you did it again, or things you would recommend to others who might want to do the same thing on their unit. BACKGROUND Describe the problem in more depth. For example this might include how, when, and where does the problem occur, what is the history of the problem, how widespread is it, what are contributing factors, what is the impact and why is it important? DISCUSSION/NEXT STEPS Provide a short summary of the project. Then discuss the importance of it and/or recommendations or next steps. For example: Would you recommend spreading the improvement to other areas? Do you need to do more work in your area? What could be done next to make the change more permanent or even better? OUTCOME/IMPACT GOALS Type in one to three goals you set out to accomplish. They should be specific, measureable, and achievable. Describe the outcome/impact of your project. What actually changed as a result of your project? For example, if you did a survey what were the results? If you did education, how many people did you reach? If you did a followup survey or test, was there improvement? TEAM MEMBERS List all your team members including credentials and titles. Include a picture if desired.
