2018 -2020 Community Health Improvement Plan • Developed through Truckee Meadows Health Communities • 3 focus areas have been identified – Housing – Nutrition/Physical Activity – Behavioral Health • Action plans being developed for each focus area
2018 -2020 CHIP • Each “Action Plan” includes: – High level outcome indicator(s) – High level goal(s) – SMART objectives • Strategies and tactics to accomplish the objective • Owner of the strategies/tactics and a target date for completion
2018 -2020 CHIP • DRAFT Potential Goals and Objectives for the Behavioral Health focus area have been developed • Input being sought – Realistic and appropriate goal? – Not fully “SMART” yet • TMHC Board will determine final approval of “Action Plan”
2018 -2020 CHIP • Goal 1: Increase funding for housing and community case management for the severely mentally ill (SMI). • Objectives (SMART Format): – By September 1, 2018 identify and advocate for alternative funding models for housing SMI. – By September 1, 2018 identify best practices for incorporating community case management** for SMI receiving housing assistance.
2018 -2020 CHIP • Goal 2: Assess and address current status and need for Behavioral Health services in Washoe County • Objective (SMART Format): – Develop a strategy to collect and disseminate information related to the annual statistics on the number of active Behavioral Health providers, clients served and types of payment accepted in Washoe County. – Identify gaps in service and access for those needing behavioral health services in Washoe County. – Develop strategies and advocate for policies to address gaps and needs identified.
2018 -2020 CHIP • Goal 3: Reduce depression and suicidal behaviors in adolescents • Objective (SMART Format): – Increase mental health screening of 7 th grade students in Washoe County using a standardized screening tool and provide appropriate referral to care. – Increase adolescents’ access to protective factors like recreational/extracurricular activities and anti-bullying effort.
2018 -2020 CHIP • Goal 4: Reduce depression and suicidal behaviors in adults and seniors • Objective (SMART Format): – Pilot or expand an existing behavioral health screening using a standardized screening tool, which includes an evaluation for depression, within primary care settings. – Develop a marketing campaign targeted towards adults to increase general awareness of signs of depression. – Identify best practices to support to patients recently discharged after a suicide attempt. – Develop a strategy to implement best practices identified to support patients recently discharged after a suicide attempt.
2018 -2020 CHIP • Goal 5: Increase prevention and treatment of Opioid abuse • Objective (SMART Format): – Secure regional funding and implement education and prevention strategies for opioid misuse – Secure regional funding and implement harm reduction strategies and expand medically assisted treatment for opioid addiction
