Eligibility Paperwork • Go to preble. gbaps. org/athletics for all eligibility forms 1. Activities/Eligibility Form – every player, every year 2. Physical – every player, every other year 3. Health Information Disclosure Authorization (HIPAA) – every player, every year 4. Paper work due July 8 th, 2016 to main office 5. Can’t participate without paperwork completed

PSA/Summer Workouts • Sign up for Sports Academy @ http: //preble. gbaps. org/ • Monday – Friday – 6: 30 – 8: 30 (11 th & 12 th grade lifting) – 8: 30 – 10: 30 (9 th & 10 th grade lifting) – 10: 30 – 12: 00 (Freshman Football) – 9: 00 – 10: 30 (3 rd – 5 th Football) – 10: 30 – 12: 00 (6 th – 8 th Football) – Workouts Available for Pick Up if out of town over summer
Contact Days - Freshmen • WIAA allows five “contact days” with players during offseason • Freshmen – July 11 th – 15 th – 8: 00 – 10: 00 – @ Preble – $30 • All participants receive camp t-shirt & snacks – No Sports Academy this week

Contact Days – JV & Varsity • Tuesday, July 12 th – Thursday, July 14 th – 7: 30 – 11: 00 @ Preble – Monday, July 11 th & Friday, July 15 th are Sports Academy days • July 18 th & 19 th – 9: 00 am – 8: 00 pm @ St. Norbert – Must sign waiver form from St. Norbert – $70 (includes lunch & dinner & instruction from St. Norbert coaches both days)

Attendance Guidelines 1. Contact days & Sports Academy encouraged but not mandatory. Mandatory practices start August 1 st. 2. Players expected to attend all practices & games. 3. Consequences for unexcused absences include loss of playing time & dismissal (please see player expectations for details). 4. Players must ride bus home after games unless transportation waiver is completed 5. Proactive communication by players.
Equipment 1. All players will be issued high quality helmets, uniforms, & shoulder pads (made possible through fundraising efforts) 2. Available for purchase from Preble Football: – Belt – $2 – Mouth guard – $1 Purchase from another source: – Integrated girdle • Includes hip, tailbone, & thigh pads – Cleats • Limited supply of equipment located in Hive – Football does not turn anyone away because of inability to pay
Fundraiser • All players expected to participate in fundraiser • Necessary in order to provide equipment, coaches, & uniforms • Festival Foods/Grand Central Station cards – 2 Ultimate Car washes valued at $12 each – Six 5% off coupons with $75 purchase from Festival Foods • Kickoff: August 3 rd after practice • Blitz Night: August 15 th @ 6 pm • Goal is for every player to sell 20 cards – 30 = custom sweatshirt
Communication You Can Expect From Coaches 1. Locations and times of all practices & contests to the best of the coach’s knowledge. 2. Team requirements such as player fees, equipment, and off-season conditioning. 3. Protocol regarding injuries & player safety. 4. Behavior expectations including consequences for failure to attend practices and/or games (Click HERE to see team rules & attendance guidelines).
Communication Coaches Desire from Players, Parents, & Guardians 1. Questions & concerns expressed directly to the coach. 2. Notification of any schedule conflicts regarding practices or contests. 3. Issues pertaining to the emotional or physical well-being of your child. 4. Ways to help your child succeed & improve academically and athletically.
Player Communication Please encourage your child to meet with their coach to address concerns as a first step. This conversation is especially important if there are concerns regarding playing time.
