INCIDENT REPORTING I MUST Be pro-active and report all unsafe situations Not reporting an unsafe situation could result in an accident occuring Every one of us has a duty to report any unsafe situations It is fundamental to reduce the Bird pyramid basis Propose alternative ways to solve unsafe situations Our HSE involvement includes submitting well-founded proposals for preventing unsafe situations As professionals we are in the best position to propose suitable actions to the HSE Department January 10, 2022 | 9

INCIDENT REPORTING January 10, 2022 | 10 I MUST Explain what makes the situation unsafe to the person concerned before reporting it Reporting an unsafe situation without explaining why it is dangerous means the person concerned continues to be at risk Remember that I can report all unsafe situations whatever the hierarchical position of the person concerned Hierarchical levels must not affect safety behaviour Nobody will blame you for having prevented a potential accident

INCIDENT REPORTING I MUST NOT Think that if I report an unsafe situation I will be considered a “trouble maker” By reporting an unsafe situation you are not making trouble, you are preventing it Never forget: the more unsafe situations we report, the fewer accidents that will happen Think that it is not important Everyone has his or her own “safety vision” What is important to you may not be important to a colleague, but reporting an unsafe situation will help everybody work safely January 10, 2022 | 11
