Presenting Issues that Applied 92% of contacts involved a mental health concern; the most common problems were family or relationship conflict, followed by suicide or self-harm, substance use, homelessness, aggression and medical concerns… yet only 7% of these contacts resulted in an Emergency Department referral. The overwhelming majority were able to resolve the issue without accessing higher level care resources.

In Summary • More than 2, 700 face-to-face and phone contacts and reaching at least 479 unique individuals between 5/2018 – 6/2019 • 51% show immediate improvement while over half of services are in efforts to proactively engage and develop relationships for longer term impact • Community Outreach has created a deeper alliance between law enforcement, social services, behavioral health and community members.
