5 Funding Mechanism: Contract Amendment Technically not a stand-alone grant Following application period, contract amendments will be made in Summer 2021 CDPH will review applications received and provide amendment documents that are ready for signature LHJ Board of Supervisors and internal review processes LOHPs who accept funding will amend budgets and workplans September 2021 – July 2022 implementation period California Department of Public Health Center for Healthy Communities Office of Oral Health

Component 1: School-linked Program Model: Referral Management and Care Coordination Goal: Increase children receiving dental screening and referral Formal community-clinical linkages between LOHPs, local clinics, dental providers, and one or more schools Hardware and software purchases, training, onboarding, and technical support Work with CHDP and other local programs to establish a dental home California Department of Public Health Center for Healthy Communities Office of Oral Health 9
10 Component 1: Allowable Purchases Allowable hardware: Laptops/tablets, intraoral cameras, networking tools Allowable software: Must support electronic referrals Onboarding, training, technical support, software configuration, EHR integration OOH will host info sessions for LOHPs applying for Component 1 following funding award California Department of Public Health Center for Healthy Communities Office of Oral Health
Component 2: Evidenced-based support for School-based sealant programs Median 60% decrease in cavities on the top and bottom surfaces of molars and premolars among children 6 to 17 years old Applying sealants in school-based or -linked programs was found to be effective among children at different risk of tooth decay and in families of varying economic means. Six studies provided information on the economic efficiency of school-based or school-linked programs. 15 Which Component(s) to Apply For Component 1: Referral Management and Care Coordination Software and Hardware Consider applying if you aren’t currently using a software system to support community clinical linkages Some costs as low as $50 - $100 per license May actually be very practical for smaller jurisdictions Provider onboarding and training needs should be considered California Department of Public Health Center for Healthy Communities Office of Oral Health
16 Which Component(s) to Apply For (2) Component 2: Equipment Support Consider applying if availability of equipment and start-up costs is prohibiting school-based preventive dental services Investing in equipment may help secure partnerships Partnering with local schools, FQHCs, Medi-Cal Dental providers, and community-based organizations Securing MOUs will be key California Department of Public Health Center for Healthy Communities Office of Oral Health
